Contact Forms & CRM Integration

Capture leads directly from your website with embedded contact forms. Every submission automatically creates a lead in your CRM and notifies your team.

Adding a Contact Form to Your Website

  1. In the website editor, click + Add Section.
  2. Select the Form section type.
  3. Choose an existing form from your form library, or create a new one.
  4. The form is embedded directly on the page.
  5. Publish your website to make the form live.

Creating a Custom Form

  1. Go to Website → Forms (or create from the section editor).
  2. Click + New Form.
  3. Add fields: Name, Email, Phone, Message, and any custom fields you need.
  4. Set which fields are required.
  5. Configure the confirmation message shown after submission.
  6. Set the notification email address — where submission alerts are sent.

What Happens When Someone Submits

  1. A new lead is automatically created in your CRM (People → Leads) with all form data attached.
  2. An email notification is sent to your gym's configured notification email address (via Resend).
  3. The submission is logged and viewable from the form's Responses tab.
  4. If automations are configured, the new lead can trigger a welcome flow (e.g. "New lead → Send info email → Wait 2 days → Follow-up").

Viewing Responses

All form submissions are stored and accessible from Website → Forms → [Form Name] → Responses. You can view individual submissions, filter by date, and see which leads have been converted.
